Adds initial structure for the accessibility-ready guidelines.

This PR closes: #164

It adds two new sections under Standards and best practice:

  • Documenting Accessibility
  • Theme accessibility guidelines

And one new section under Testing WordPress themes:

  • Testing WordPress themes for accessibility

Hey @amberhinds and @joedolson
What a tremendous amount of excellent work!

I propose this goes in as is, you have worked so long on this, let's just merge it.

No show stoppers, but for the future I would propose:

  • Let testers take all steps and see if they understand everything, maybe a good task for a contributor day. It's a lot.
  • Accessibility Conformance Reports: I miss WCAG-EM reporting, as we use in Europe, I will ping Hidde de Vries about this, for his input.
  • With the guidelines: add links to the related topics on wpaccessibility.org, as soon they exist.
  • Uniform the Notes as we do with the other docs.
  • Maybe add the success criterion number, as we do with the related WCAG guidelines in the topics.
