Precompute line start offsets and slice source code around annotations and patches. This prevents passing entire large files to the `annotate_snippets` library, significantly improving rendering performance. Additionally, refactor `byte_offset_to_line_col` to leverage these precomputed line starts, enabling O(logN) lookup for line/column information.
