Resolves https://jira.hl7.org/browse/FHIR-55247 by adding constraint requiring MessageHeader.response.details be populated when Messageheader.response.code is fatal-error or non-fatal error.
Resolves https://jira.hl7.org/browse/FHIR-55247 by adding new code of non-fatal-error to response-code. Changes binding of MessageHeader.response.code from required to extensible.
