[iOS] Session title generation fails (Streamable HTTP error) and manual rename is unavailable

Summary

First, thank you very much to the Happy development team for all your work and continuous improvements.

On iOS, session title handling appears to be broken with this error:

Streamable HTTP error: Error POSTing to endpoint

Current Behavior

1. Codex cannot auto-generate a session title.
2. I also cannot manually rename the title of a specific session.

Expected Behavior

1. Fix automatic title generation for sessions.
2. Allow users to manually edit/rename the title of an individual session.

Why This Matters

When titles cannot be generated or edited, session organization becomes difficult, especially across multiple ongoing conversations.

To make triage easier, this report can likely be tracked as two workstreams:

1. Auto-title generation failure (Streamable HTTP error: Error POSTing to endpoint) — likely related to MCP/title transport reliability (see [Bug] change_title MCP tool silently fails — HTTP 500 from StreamableHTTPServerTransport #768).
2. Manual rename support on iOS for a specific session title — feature parity/fallback request (related to Add ability to edit session titles from the web UI #871, currently web-focused).

If preferred, I am happy to keep this issue as iOS tracking and follow whichever canonical issue numbers you want.
