Assistant (Anthropic, sonnet 4.5, .ipynb) unable to sucessfully edit existing notebook files

when using a new install today
created a new python notebook for data visualization
assistant created code to import data, shape data, visualize data, in multiple notebook cells
when run, data shaping cell threw errors.
assistant could understand the error, attempted edits to fix.
accept changes modal shown with line edit totals +n-n, but changes to the file were not visible even though modal was presented
accept changes has no effect: clicked accept but changes not present in file, and not visible even though modal was presented.

assistant at one point during troubleshooting, created and subsequently edited a python file successfully.
Modals and changes in file appeared styled as expected.
no idea how that was different. LLM, interestingly, has concluded it cannot edit JSON files.

Q: is this a config error, or should I open a defect?
