Automatically Append Tool Descriptions

Currently, when adding a new tool, its description must also be written manually in both README.md and README.ja.md. However, this step is often forgotten.

Since the tool definitions in the source code already include descriptions, this issue can be resolved by automatically reflecting that content in the READMEs. Support for the Japanese version is also necessary, but this can be handled by including both languages in the source code, for example (this is open to discussion).

Once automatic generation is in place, we should remove the instruction to manually add tool descriptions to the README from CONTRIBUTING.md.
