Describe the bug
When creating an item in the iOS control center for a switch, after configuring the item it seems the setting does not get saved. Instead of showing the actual switch name it shows a default "switch" string and the configured icon also does not show up. When entering edit mode again, all settings are there.

Of course, the switch itself also have no function then. This is also the case for e.g. "light" items. In contrast - a simple "open switch" item works perfectly fine.
User is a standard user, no admin.
On another phone (same model, os version and companion app version) it just works as expected.
Also can't exclude its an OS bug, but could not find any different setting to the working phone.
What I already tried / checked
- Phone restart
- Delete companion app and reinstall
- Give user Admin rights
- Checked permissions for background updates/activities and more - all given
- Searched for similar issues here on GitHub, could not find any
To Reproduce
Only reproducible on one phone here. There its always like this. No way to create a control center item for a switch that works.
Expected behavior
Created items in control center should just work
