Add Module and runtime health dashboard widget

Follow-up from #315. This is intentionally deferred from the initial Dashboard and Attention delivery so the first release stays focused on Attention, Workflow Portfolio, and Workflow Run Health. Refine scope and acceptance criteria during triage.
