Update revocation benchmarks on lists that do not use hash identifiers

The benchmarks for indexed revocation lists are a bit pointless like this, would be good to have also a comparison with blinded revocation lists that do not use a credential hash as index.

Also would be useful to make it more clear in the paper that a hash index should only be used with `H(m:nonce)` credentials.
